ABSTRACT

Apatinib mesylate is a novel small molecule monoclonal antibody. Its mechanism of action is through highly selective com-petition for intracellular vascular endothelial growth factor receptor-2 (VEGFR-2) ATP binding sites and blocking downstream signal transduction thereby inhibiting neovascularization of tumor tissue. Hand-foot skin reaction (HFSR) is one of the common adverse reac-tions of apatinib, and it often affects the quality of life of patients; this leads to a reduction in the use of targeted drugs , which nega-tively affects the efficacy of anti-tumor therapy. The conventional western medicine treatment for HFSR comprises urea and glucocorti-coid topical preparations. Clinical treatment is often based on redness, ulceration, scab, and other skin manifestations; a combination of Chinese and western medicines can improve clinical outcomes. This article reviews apatinib-related HFSR and the relevant progress in treatment strategies in order to provide a clinical reference.

ABSTRACT

Objective To optimize the extraction and β-cyclodextrin clathration process of volatile oil in Tianma-Shouwu tablet.Methods The extraction of volatile oil was optimized through the orthogonal design L9(34) with the volatile oil extraction amount as assessment index. The saturated aqueous solution preparation of clathrate form was used. The volume of β-cyclodextrin and volatile oil, clathration temperature, clathration time was investigated by central composite design-response surface methodology, with the score of volatile oil inclusion ratio and utilization ratio as the index. The results were fitted by second-order polynomial equation, and then the best clathration process was determined. Results The optimization extraction process of volatile oil in Tianma-Shouwu tablet was as follows:to add 8 times water,immerse 30 minutes,steam distill 6 hours;and the best clathration process was as follows: the ratio of β-cyclodextrin and volatile oil was 9.50:1, clathration temperature 43 ℃, and clathration time 4.2 hours. Conclusions The optimized extraction and clathration process were reasonable, stabled and repeatable.

ABSTRACT

Objective The aim of this study was to investigate the mechanism underlying pruritus by comparing the epidermal growth factor receptor inhibitor(EGFRI)-erlotinib mouse model with the substance P(SP)-induced pruritus mouse model. Methods Two randomized groups of mice were treated with erlotinib or SP to induce pruritus. Behavioral and skin manifestations were observed. Pathological images and neurokinin 1 receptor(NK-1R)expression of the skin were determined. Concentration of interleukin(IL)-31, IL-33, histamine, leukotriene B4, and SP was analyzed by enzyme-linked immunosorbent assay. Nitric oxide was analyzed by colorimetry. Results Transient pruritus induced by erlotinib appeared 2 to 5 days after treatment. In contrast, continuous pruritus was observed during the first hour, but was then gradually relieved. These two shared similar scratching behavior. Concentration of neurotransmitters showed similar trends in changes among the erlotinib group and SP group. Immunohistochemical expression was also consistent between the erlotinib group and SP group. Conclusions Erlotinib-associated pruritus is related to release of signaling factors through the SP/NK-1R signaling pathway.

ABSTRACT

In the past decades, the advent of molecular targeted agents has been a considerable breakthrough for cancer patients, ex-cept for traditional operation, chemotherapy, and radiotherapy. As a novel therapeutic method, molecular targeting treatment is im-portant in the field of medical oncology, in which epidermal growth factor receptor inhibitors (EGFRIs) have been widely used as target-ed agents. Dermatological toxicities are the common side effects associated with the EGFRIs. Diarrhea, weakness, and pneumonia can be observed. EGFRI-induced dermatological toxicities may disrupt the health-related quality of life and cause anticancer treatment dose adjustments or discontinuance. The dermatological toxicities mainly involve rash acneiform, pruritus, xerosis, paronychia, hair change, and skin hypersensitivity. Treatment recommendations from guidelines include antibiotics, corticosteroids, and antihista-mines, but their clinical therapeutic efficacy have not been proven. Therefore, oncologists and dermatologists are investigating effec-tive medication. This article reviews the advances in the study of the clinical manifestations and drug therapies of EGFRI-induced der-matological toxicities to provide reference for clinical practitioners.

ABSTRACT

Objective To know the preventive effects of using hammock firmness sheets for pa-tients with pressure ulcer.Methods Divided 98 patients into the application group (31 cases), the treatment group(30 cases) and the control group(37 cases) according to themselves condition.Hammock firmness sheets was used in the application group, while the routine preventive method of pressure ulcer was used in the control group.To know the preventive effects of pressure ulcer betweent the two groups.The patients in the treatment group was patients had pressure ulcer, hammock firmness sheets combined with local massage were used for them, observed the treatment effects for them.Results The condi-tion of preventive effect of pressure ulcer in the application group was better than those of in the control group.The cure rate of pres sure ulcer in the treatment group was 90%.Conclusions The hammock firmness sheets can prevent pressure ulcer effectively.

ABSTRACT

Objective To explore the constraint effect of self - made binding sheets to restless patients. Methods Using soft canvas and thick qualitative cotton for sheets making. 100 cases of Neurusurgery JCAHO into patients of level 2~3 were, randomly divided into 52 cases of obeservation group and 48 cases of control group, then use self - made binding sheets and traditional binding sheets respectively for the purpose of protective constraint. Re-sults The interference treatment and skin damage of the observation group, and the psychological pressure of nursing staff were significantly lower than those in control group (P < 0.01). Conclusions With self - made binding sheets the patients have integrity constraints, and the result is better than the traditional one.
